چکیده
BACKGROUND: Low-dose macrolides (LDM) are anti-inflammatory agents with antineutrophilic activity, but patient selection for LDM therapy in treating chronic rhinosinusitis (CRS) is controversial. This study aimed to assess factors which predict responders. METHODOLOGY: A prospective cohort was performed. Patients CRS received roxithromycin (150 mg) once daily 12 weeks. Nasal secretions and serology were collected. Nine predictors response assessed: nasal secretion IgE, IL-5, serum eosinophils, neutrophils, polyps, asthma, allergy, aspirin hypersensitivity, using receiver-operating curve analysis multivariable logistic regression. Macrolide responders those sino-nasal outcome test-22 improvement, symptoms visual analogue scale decreased ≤5, no rescue medication. RESULTS: One hundred patients (mean age 47.4±14.1 years, 45% male) enrolled. Univariable regression showed local total IgE less than 5.21; eosinophils 2.2% associated macrolide response. Multivariate models maintained an independent association response, ability discriminate between non-responders of 63%. Serum neutrophil, polyp, hypersensitivity CONCLUSIONS: Low level the not serum,

Chronic rhinosinusitis is a common disorder of chronic inflammation of the upper respiratory tract. It is associated with significant symptoms and impairment of the quality of life of sufferers. Despite recent advances in the medical and surgical management of chronic rhinosinusitis, there remains a population of patients who fail to obtain relief from their symptoms. Chronic inflammation of th...

We studied the immunological and histopathological factors that affect the prognosis of chronic rhinosinusitis under long-term low-dose macrolide therapy. Sixteen patients with chronic rhinosinusitis were given 200 mg clarithromycin or 150 mg roxithromycin orally once a day without other concurrent treatments for 2-3 months. Measurement of the serum IgE level, blood cell count and differential ...
متن کاملChronic GVHD: predictive factor for rhinosinusitis in bone marrow transplantation.
INTRODUCTION Bone marrow transplantation (BMT) is a treatment option for hematological diseases and immunodeficiency. It is frequently used today. BMT predisposes patients to upper airway infections and its complications, such as rhinosinusitis (RS). Chemotherapy, radiotherapy, viral infections, antibiotic therapy, graft versus host disease (GVHD) are rhinosinusitis predisposing conditions. A...
متن کاملFactors affecting revision rate of chronic rhinosinusitis
OBJECTIVE Chronic rhinosinusitis (CRS) is a variable multifactorial disease. It can be divided into forms with nasal polyps (CRSwNP) and without (CRSsNP). Sinus and/or nasal polypectomy surgery are considered if maximal conservative treatment is insufficient. The predictive factors of the need of revision surgery comprise mostly the CRSwNP phenotype and are not fully understood. STUDY DESIGN ...
